PR, Age 50yrs.
She was suffering from one sided headache. She had severe headache from many years and it used to last for about 3-4 days.

Case Taking:-
She said that whenever she has an attack of migraine, it is very severe and it lasts for 3-4 days. It is accompanied with nausea and vomiting. It is ameliorated by covering the head but is aggravated in open air. The thing which she frequently told me was that she had to cover her head even in summers. I asked her, “What do you want to do during migraine?”
She replied that she wants to close her eyes and be alone. She does not want to talk to anyone. She does not want to watch television. During migraine she cannot stand light. She draws the curtains and sits alone inside a dark room.

Analysis: Dr. M.L.Sehgal has taught us to understand the soul of the case i.e. what is his inner feeling, how he expresses and why he says so. We should not convert every version or whatever the patient says into rubrics. Rather we should try to understand the basic theme of the case. We should try to find out what a patient wants. In this particular case, three things were very important:

1. She wants to lie down with eyes closed. (Indifference lies with eyes closed)

2. She wants darkness (Dark desires during headache)

3. She feels chilly and wants to cover her head with a cloth. (Chilly, covers her head)

In this case, the core of the case was taken into consideration and she got instant relief. We have to consider only present, predominating and persistent symptoms. In this case I did not take the rubrics quiet wants to be and disturbed, averse to being because she expressed her feeling in another way. If a well selected medicine is chosen, relief is instant.
